Referring Doctor Portal

Retina and Vitreous Consultants of Wisconsin aims to streamline referral of patients through a user-friendly experience.  We encourage referring doctors to submit referrals electronically, but still offer fax submission as an option.  We also have a direct doctor phone line accessible via our main number menu to minimize waiting time for our referring doctors

Download and fill out your forms, then fax them to 414-778-3446.
